The Authorization Form Wizard functions best with Internet Explorer 5.1+ and Netscape 6.0+. If you cannot save your data, please try using one of these browsers.

At any time, you may click the "View Form" link at the top of the page to see the current state of the authorization form.

You can save the authorization form you are working on at any time by clicking the "Save Session" link and come back to finish it later.

If you want to resume a saved session, click on the "Resume Session" link.

If you are trying to save the web wizard answers entered on a page, you must first click the "Next" button and then go on to the next page. The page you are working on will not be saved until you go on to the next page.

When you view the authorization form, each completed section will have a link to its corresponding wizard page. You may click on the text, you entered, to go back to that particular section and make corrections.

When you finish your authorization form you can go back to a previous page and make corrections if you are not satisfied with the finished product.

When you finish your authorization form you can save it to a word processing program where you may spell check and edit. You can do this by following the directions at the end of the program or simply cut and paste as you normally would.
